1. As the student, legal parent or guardian of the student, I understand that San Diego Danceworks, occasionally uses photographs of classes/recitals/productions for publicity purposes.  

2. By signing this form I give San Diego Danceworks my consent to release photographs and video of student, for publicity purposes. (examples: Facebook, Twitter, Print advertisements, webpage)

As the student, legal parent or guardian of the student, I assume full responsibility for any and all damages, injuries, sickness, or losses that I may sustain or incur, if any, while participating in activities at San Diego Danceworks. I release and hold harmless San Diego Danceworks, its owners, instructors, students and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, sickness, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of San Diego Danceworks, its owners and operators or in route to or from any of said premises.

Face masks are required for all dancers, parents, guests and staff while inside the studio.

We will inform families of dancers who have close contact with another dancer who tested positive for covid.

We are following San Diego County Unified School District Decision Tree for Close Contact and Quarantine Measures.

Please do not attend classes if your dancer, or anyone in their household, has symptoms such as a stuffy nose, sore throat or fever and consult with your physician.

The undersigned gives permission to San Diego Danceworks, its owners, instructors, and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

I recognize that San Diego Danceworks Online Classes require physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand and acknowledge that San Diego Danceworks Online Classes by their very nature, carry with them certain inherent risks that cannot be eliminated. I understand and acknowledge that, regardless of the care taken by San Diego Danceworks, San Diego Danceworks cannot guarantee my personal safety, health or well-being, including illnesses (e.g. COVID-19) in which I could be exposed while participating in San Diego Danceworks Online Classes. I acknowledge that when participating in San Diego Danceworks Online Classes, there is always a possibility that injuries may occur.
I understand and acknowledge that damage to my personal property or valuables may occur as a result of my participating in San Diego Danceworks Online Classes, and I assume full responsibility for all damage to personal property and/or valuables, whether such property is owned by myself or by another person or entity, that may occur as a result of my participating in San Diego Danceworks Online Classes. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any San Diego Danceworks Online Classes. By signing below, I acknowledge that I have received a physical examination and have been given my physician's permission to participate in San Diego Danceworks Online Classes or I have knowingly chosen to not obtain my physician's permission prior to participating in San Diego Danceworks Online Classes.
